Job SummaryWe are seeking a skilled Nurse Practitioner to join our team as a PRN/Part Time Home Risk Assessment Specialist. In this role, you will conduct comprehensive health assessments, medical history reviews, and provide health education to patients in their homes. You will work collaboratively with physicians, case managers, and other healthcare professionals to ensure seamless care coordination and optimal patient outcomes.
Responsibilities- Conduct adult and geriatric assessments, medical history reviews, and provide health education to patients in their homes
- Collaborate with physicians, case managers, and other healthcare professionals to ensure seamless care coordination and optimal patient outcomes
- Work in a variety of settings, including home visits, skilled facility visits, and telehealth visits
- Provide services that meet the needs of at-risk health plan members, including those with complex medical conditions and social determinants of health
- Document patient information accurately and thoroughly, in accordance with Matrix Medical Network's guidelines and protocols
- Master's degree in Nursing or related field
- Current RN and NP licensure in state of practice, with prescription authority or ability to obtain
- Board certification by AANP, ANCC, or AACCN in a Matrix-approved specialty
- Minimum of 1 year experience as a Nurse Practitioner
- Strong computer skills and familiarity with electronic health record software
- Excellent verbal and written communication skills
- Ability to travel to patient locations, with valid driver's license and adequate automobile insurance
