Job Description
We are seeking a skilled Home Health Nurse to join our team. As a key member of our patient care team, you will be responsible for coordinating and overseeing all aspects of patient care in the comfort of their own homes. Your duties will include:
- Developing and implementing personalized care plans tailored to each patient's unique needs.
- Collaborating with multidisciplinary teams to ensure seamless care delivery.
- Providing education and support to patients and their families, empowering them to take an active role in their care.
- Performing comprehensive physical assessments and monitoring vital signs to inform treatment decisions.
- Managing medications, administering injections, and providing other necessary interventions.
- Supervising licensed practical nurses (LPNs) and home health aides (HHAs) to ensure quality care is delivered.
Requirements
To succeed in this role, you must have:
- One year of clinical experience as a Registered Nurse (RN).
- A current RN license, specific to the state(s) you are assigned to work.
- Current CPR certification.
- A valid driver's license, reliable transportation, and liability insurance.
