Key Responsibilities:- Assess, plan, and implement individualized care plans to meet the medical and health care needs of residents.
- Administer medications and ensure accurate documentation of services.
- Participate in the admission process and develop plans to ensure medical follow-up.
- Provide special instructions to residents and home staff related to ongoing medical conditions.
- Document accurate and ongoing assessment of services in accordance with Agency policy and requirements.
- Graduate of an approved school of nursing.
- Two years' experience working with the developmentally disabled, preferred.
- Current New Jersey RN License required by Statutes.
- Must hold a valid driver's license.
- Must successfully complete and obtain/maintain all certifications as required by Elwyn NJ and regulatory agencies.
