Pediatric Case Manager Job Description

Job Summary:

Pediatric Home Healthcare is seeking a highly skilled and compassionate Pediatric Case Manager to join our team. As a Pediatric Case Manager, you will be responsible for ensuring the highest quality of patient home care through direct case management.

Key Responsibilities:

  • Perform and document pediatric patient assessments
  • Ensure documentation submitted to insurance is thoroughly completed
  • Conduct Skills Reviews for field nurse applicants
  • Monitor QAPI program and ensure appropriate corrective measures are performed
  • Meet mandatory continuing education requirements
  • Promote and educate regarding the concepts of infection control and standard precautions

Requirements:

  • Graduate of an accredited Diploma, Associate, or Baccalaureate School of Nursing
  • Current and valid Registered Nurse license in the state of Texas
  • Minimum 2 years' experience in pediatric, NICU or home healthcare beside nursing
  • Minimum 1 years' experience in pediatric home healthcare is strongly preferred
  • Valid Texas driver's license and reliable vehicle with valid and current proof of liability insurance
  • Current Healthcare Provider BLS from American Red Cross or American Heart Association (or subsidiary)
  • Strong English language skills; bilingual in Spanish preferred

Pay: $69,000.00 - $80,000.00 per year
