Palliative Care Specialist Opportunity at Vitas Healthcare

Job Summary:
We are seeking a dedicated and compassionate Registered Nurse to join our interdisciplinary team at Vitas Healthcare. As a Palliative Care Specialist, you will play a critical role in providing high-quality, patient-centered care to individuals and their families facing terminal illness and bereavement.

Key Responsibilities:
The successful candidate will be responsible for coordinating care, ensuring continuous assessment of patients and families, and implementing the interdisciplinary plan of care. Key responsibilities include:

  • Case management and supervision of nursing staff
  • Initial and ongoing comprehensive assessments to provide palliation of end-of-life symptoms
  • Identification and utilization of resources, referrals, and consultations as needed
  • Effective communication and shift reports through written, verbal, and body language
  • Participation in patient care meetings, weekly team meetings, and bereavement support

Requirements:
To be successful in this role, you must have a current nursing license in the state where the Vitas program is located, a minimum of two years of nursing experience in hospice, home health, or community health, and the ability to work independently and as part of a team.

Work Environment:
The Palliative Care Specialist will work in a fast-paced, dynamic environment, providing care in patients' homes, hospitals, and other care settings. You must be able to sit and stoop, kneel, and crouch, and have the ability to assist in lifting and transferring patients as required.
