About the Role

We're seeking a skilled Physical Therapist to join our Home Care team. As a Physical Therapist, you will provide in-home physical therapy services to patients with neuro and/or musculoskeletal disorders, under the direction of a physician.

Responsibilities
  • Assess and treat patients in their homes, utilizing a variety of techniques to improve mobility and function.
  • Develop and implement individualized treatment plans to meet the unique needs of each patient.
  • Collaborate with healthcare teams to ensure seamless care and communication.
  • Document patient progress and maintain accurate records.
  • Provide education and support to patients and their families to promote independence and self-care.
Requirements
  • Graduate of an accredited physical therapy program.
  • Current state license and valid driver's license.
  • Certifications: CPR certification.
  • Good assessment and treatment planning skills, with the ability to work independently and handle emergencies without assistance.
  • Excellent communication and documentation skills, with the ability to work effectively with change and adapt to new situations.
Preferred Qualifications
  • Two years of experience in acute care or home care.
