Your Responsibilities

\r
  1. \r
  2. Conduct initial and ongoing comprehensive assessments of patients and families to develop personalized plans of care.\r
  3. Utilize health assessment data to determine nursing diagnoses and administer medication and treatment as prescribed by the physician.\r
  4. Counsel patients and families on meeting nursing and related needs, while maintaining accurate clinical notes.\r
  5. Communicate effectively with physicians and participate in interdisciplinary group meetings to address patient and family/caregivers' medical and emotional needs.\r
  6. Evaluate patient progress and adjust plans of care as necessary to ensure compliance with state and federal regulations.\r

Your Qualifications

\r
  • \r
  • A graduate of an accredited nursing school with current licensure to practice in the state of operation.\r
  • At least one year of nursing experience, with three or more years preferred.\r
  • Experience in hospice or similar settings is desirable, but not required.\r
  • Demonstrated knowledge of the hospice philosophy of care and commitment to clinical excellence.\r
