Key Responsibilities
As an RN Care Manager at Strive, you will play a pivotal role in supporting patients with Chronic Kidney Disease (CKD) and End-Stage Renal Disease (ESRD). Your responsibilities will include:
- Collaborating with the care team to develop personalized care plans that address both clinical and non-clinical needs.
- Conducting regular assessments, both virtually and in-person, to identify any medical or social issues that may arise.
- Proactively reaching out to patients and clinics to monitor their status and provide assistance as needed.
- Managing a diverse patient panel, ensuring compliance with NCQA standards for complex case management.
- Communicating effectively with the care team and families, providing education and resources to enhance patient understanding of their health.
- Acting as a liaison with dialysis facilities to ensure smooth transitions in care.
- Utilizing data platforms to generate reports that identify trends and areas for improvement.
- Delivering culturally sensitive care that meets the diverse needs of our patients.
- Applying clinical judgment and analytical skills to provide high-quality care.
- Associate's degree in nursing from an accredited institution.
- Current Registered Nurse (RN) license in the applicable state.
- At least 3 years of relevant experience or education.
- Current BLS or CPR certification.
- Reliable transportation for patient visits.
- Ability to provide in-person care, including physical activities such as standing and lifting.
- Access to reliable internet with specified minimum speeds.
- 3+ years of experience in nephrology, emergency, ICU, or complex case management nursing.
- Certification as a Case Manager (CCM) is preferred.
- Proficiency in Microsoft 365 Suite and electronic medical records.
- Proven ability to build strong relationships with patients and families to foster engagement.
- Experience in proactive patient outreach to assess needs.
- Familiarity with electronic medical records and care plan management.
