We are looking for a skilled and caring RN to join our team. As a Registered Nurse, you will be responsible for visiting and assessing patients, creating and managing patient plans of care, and educating patients and their families on proper care and management.

Responsibilities:
  • Visit and assess patients to complete comprehensive assessments and ongoing evaluations
  • Chart all clinical documentation in our electronic health record (EHR) software
  • Create and manage patient plans of care
  • Educate and instruct patients, families, and caregivers on proper care and management
  • Case manage patient episodes or benefit periods, ensuring all cares are performed and documented
  • Provide medication education and management per physician order
  • Work collaboratively with the interdisciplinary team (IDT) and interdisciplinary group (IDG) to ensure patient goals are met and outcomes are achieved
  • Share on-call duties and maintain strong communication with patients, families, caregivers, and the IDT
Requirements:
  • Current license to practice professional nursing in the state of Utah
  • Polite and compassionate demeanor
  • Excellent observation, problem-solving, verbal, and written communication skills
  • Maintain current CPR certification
  • 1 year of Home Health or Hospice experience preferred
  • Licensed driver with an automobile that is insured, registered, and in good working order
  • Good communication and smartphone skills
  • Ability to pass a background check
