Patient Care Specialist

We are seeking a skilled Licensed Practical Nurse to join our team as a Patient Care Specialist. In this role, you will provide high-quality nursing care to patients in a skilled nursing setting, working under the supervision of a Registered Nurse.

Responsibilities
  • Administer medications as prescribed and chart patient information accurately and timely.
  • Assess patient needs and develop individualized care plans to ensure optimal patient outcomes.
  • Collaborate with the healthcare team to coordinate patient care and provide education to patients and families.
  • Monitor patient vital signs and report any changes to the RN or physician.
  • Provide ostomy care, wound dressings, and other nursing interventions as needed.
Qualifications
  • Current licensure as a Licensed Practical Nurse in the state where the hospital resides.
  • One year of inpatient medical-surgical nursing experience preferred.
  • Ability to communicate effectively with patients, families, and healthcare team members.
  • Strong organizational and time management skills.
