Job Title: Home Health Physical Therapist Assistant

Job Summary: We are seeking a skilled Physical Therapist Assistant to join our team of healthcare professionals at Interim HealthCare of New Hampshire. As a Home Health PTA, you will provide personalized therapy to patients in their homes, helping them regain strength, manage pain, and reach their goals.

Responsibilities:

  • Provide home-based physical therapy to patients who have lost mobility, strength, and functionality due to an illness or injury
  • Work as part of a home health team focused on the patient's plan of care and goals
  • Observe movements, conduct assessments, document progress, and modify care plans as needed
  • Assist patients with exercises and hands-on therapy to manage pain and increase mobility
  • Educate patients on their plan of care, established goals, exercises, and self-care
  • Coach family members on the plan of care, therapy goals, and safely assisting with exercises

Requirements:

  • Active PTA license in Maine
  • Minimum of 1 year of PTA experience, ideally in home healthcare
  • CPR certification
  • Up-to-date on all State of Maine Required vaccinations
  • Knowledge of state and federal home health regulations
  • Good clinical judgment, strong interpersonal skills, dexterity, and compassion
