Job Summary

Conifer Health Solutions is seeking a highly skilled Patient Advocate Liaison to join our team. As a Patient Advocate Liaison, you will play a critical role in ensuring that patients receive the best possible care and support throughout their healthcare journey.

Key Responsibilities
  • Screening and Eligibility: Conduct thorough interviews with patients to determine their eligibility for various government and non-government programs.
  • Financial Clearance: Perform financial clearance functions, including collections, and prepare accounts for financial assistance review.
  • Government Program Eligibility: Follow up on government application submissions to ensure timely processing and resolution.
  • Communication and Coordination: Develop strong relationships with patients, hospital staff, and community resources to ensure seamless care coordination.
  • Documentation and Record-Keeping: Maintain accurate and detailed records of patient interactions, program eligibility, and financial clearance activities.
Requirements
  • Education: High school diploma or equivalent required; minimum 2 years of experience in social services, hospital admitting, or related field.
  • Skills: Excellent communication and interpersonal skills; ability to work independently and as part of a team; proficiency in Microsoft Office and Windows operating systems.
  • Knowledge: Working knowledge of government programs, including Medicaid, Medicare, and other relevant programs.
