Job Summary

ProMedica is seeking a highly skilled and compassionate Wellness Supervisor LPN to join our team. As a key member of our Wellness Team, you will be responsible for delivering exceptional resident services that meet the health and daily living needs of our residents and their families.

Key Responsibilities
  • Provide direct resident service delivery and manage other Wellness Team Members as delegated by the Wellness Director.
  • Ensure the delivery of high-quality resident services that meet the health and daily living service needs and desires of our residents and their families.
  • Interact with residents and families in a tactful, sensitive, and professional manner to guarantee their satisfaction.
  • Collaborate with the Wellness Director to develop and implement resident service plans that meet the unique needs of each resident.
  • Conduct regular assessments of resident needs and preferences to ensure that our services are meeting their expectations.
  • Develop and maintain relationships with residents, families, and healthcare providers to ensure seamless care coordination.
  • Participate in quality improvement initiatives to enhance resident satisfaction and experience.
Requirements
  • Graduate of an accredited school of nursing.
  • Excellent command of the English language in both oral and written forms.
  • Experience in an assisted living setting and/or other elderly care setting preferred.
  • Licensed practical professional nurse in the operating state.
  • Ability to stand, walk, bend, and squat for prolonged periods.
  • Ability to push and pull objects and lift and carry up to 20 pounds, unassisted, on a frequent basis.
