About the Role

ConcertoCare is seeking a highly skilled and compassionate Care Navigator to join our team. As a key member of our clinical support team, you will play a critical role in delivering exceptional patient care and ensuring seamless communication between our care teams and patients.

Key Responsibilities
  • Manage Patient Communication: Respond to inbound calls, voicemails, and other forms of communication, applying standard guidelines to address or route messages to the appropriate care team member for follow-up.
  • Schedule Appointments: Coordinate in-home and virtual appointments, including optimizing routes for in-home appointments and minimizing drive time for our in-field care teams.
  • Support Patient Care: Complete visit confirmation and routine check-in and check-out activities for scheduled home and virtual appointments, and perform outbound calls and tasks as needed to coordinate patient care.
  • Medical Records Management: Process and track medical record requests, scan and file patient documents, and maintain up-to-date patient contact and demographic information.
  • Administrative Support: Assist with new patient registration, verify insurance eligibility, collect co-pays and fees, and process orders and referrals as needed.
Requirements
  • Education: High school diploma or GED.
  • Experience: 2+ years of experience working in a healthcare setting, including 1+ year of call center experience or equivalent work experience.
  • Certifications: Certified/Registered Medical Assistant or experience as a Medical Assistant preferred.
  • Skills: Demonstrated excellence in customer service, proficiency in telephony and medical record systems, knowledge of medical terminology, and knowledge of HIPAA rules and regulations.
