Nurse Practitioner

We are seeking a skilled Nurse Practitioner to join our team in a PRN/Part Time capacity, providing home risk assessments to our clients. As a Nurse Practitioner with Matrix Medical Network, you will have the opportunity to make a meaningful impact on the health and well-being of our patients.
Key Responsibilities- Conduct comprehensive home risk assessments, including medical history, diagnosis, and treatment planning.
- Collaborate with healthcare professionals, including physicians and case managers, to ensure seamless care coordination.
- Provide education and support to patients and their families, promoting healthy behaviors and disease management.
- Work in a variety of settings, including home visits, skilled facilities, and telehealth visits.
- Master's degree in Nursing or related field.
- Current RN and NP licensure, with prescription authority.
- Board certification in a Matrix-approved specialty.
- Minimum of 1 year experience as a Nurse Practitioner.
- Strong computer skills and familiarity with electronic health records.
- Excellent communication and interpersonal skills.
