Oncology Patient Navigator

We are seeking a highly skilled Oncology Patient Navigator to join our team at the University of Maryland Medical System. The successful candidate will work closely with our Cancer Medical Director and UM Capital Executive Leadership team to develop and implement comprehensive cancer services across our organization.
Key Responsibilities- Provide Education and Support: Educate patients and their families about cancer types and treatment options, providing expert guidance and resources throughout the care continuum.
- Coordinate Care: Assist patients in making and coordinating appointments, obtaining necessary records and test results, and communicating with physicians and other healthcare professionals.
- Assess Patient Needs: Identify clinical needs, issues, financial resources, and care goals for new oncology patients, and develop plans to address these needs.
- Develop Quality Improvement Plans: Collaborate with the interdisciplinary physician team to develop and implement quality improvement plans, tracking outcomes and reporting key quality metrics.
- Provide Customer Service: Deliver exceptional customer service, responding promptly and appropriately to patient questions and concerns, and providing assistance and support as needed.
- Communicate Effectively: Communicate effectively with patients, families, and healthcare professionals, using respectful, courteous, and professional communication protocols.
- Participate in Community Outreach: Participate in community health fairs and education events, and support the development of outreach strategies and community education initiatives.
- Education: Bachelor's degree in a related field, such as nursing, social work, or healthcare administration.
- Experience: Minimum 2 years of experience in a healthcare setting, preferably in oncology or a related field.
- Skills: Excellent communication and interpersonal skills, with the ability to work effectively with patients, families, and healthcare professionals. Strong organizational and time management skills, with the ability to prioritize tasks and manage multiple responsibilities.
