About the Role

As a Home Health Physical Therapist with Interim HealthCare, you will provide home-based physical therapy to pediatric patients who have lost mobility, strength, and functionality due to an illness or injury. You will work collaboratively with a multidisciplinary team, including an RN, LPN, CNA, HHA, OT, and SLP, to develop and implement personalized care plans that cater to each patient's unique needs.

Key responsibilities include:

  • Observing movements, conducting assessments, documenting progress, and modifying care plans as needed
  • Assisting pediatric patients with exercises and hands-on therapy to manage pain and increase mobility
  • Educating patients on their plan of care, established goals, exercises, and self-care
  • Counseling families on the plan of care, therapy goals, and safely assisting with exercises
Requirements

To be successful in this role, you will need:

  • Graduation from an accredited Physical Therapist Program and active PT license in Colorado
  • A minimum of one year of physical therapy experience, preferably in home healthcare
  • No COVID vaccination required
  • A passing background check and drug test (we are required to test for THC)
  • Knowledge of state and federal home health regulations
  • Good clinical judgment, critical thinking skills, strong communication, empathy, and compassion
