Job SummaryAveanna is seeking a skilled Registered Nurse to provide part-time home health care services in Richland Center, WI and surrounding communities. As a Weekend / After Hours Registered Nurse, you will be responsible for delivering high-quality patient care and collaborating with healthcare teams to ensure optimal patient outcomes.
Key Responsibilities- Develop and implement individualized care plans through physician orders, client input, and nursing assessment.
- Educate clients and their family members on specific care needs.
- Orient and train primary caregivers to ensure optimal functioning levels.
- Coordinate continuum of care through documentation and timely communication with physicians and caregivers.
- Provide skilled nursing care in patients' homes, utilizing skills such as phone triage, patient education, observation/assessment, wound care, infusions, catheter care, and PICC line dressing changes.
- Active RN License in the state of application.
- Valid CPR and driver's license.
- Preferred: Medicare Skilled Nursing experience, basic understanding of Oasis, and 1-year RN experience in a healthcare setting.
