Responsibilities:

  • Conduct thorough assessments of patients and develop individualized care plans to meet their unique needs.
  • Administer medications and treatments as prescribed by physicians, ensuring accurate documentation and adherence to regulatory guidelines.
  • Provide emotional support and counseling to patients and families, promoting a peaceful and dignified end-of-life experience.
  • Collaborate with interdisciplinary teams to coordinate care and ensure seamless transitions between settings.
  • Document patient progress and outcomes accurately and thoroughly, maintaining confidentiality and adhering to HIPAA guidelines.
  • Supervise and educate nursing assistants and volunteers, promoting a culture of excellence and compassion.
  • Stay up-to-date with industry developments and best practices, participating in ongoing education and training to enhance skills and knowledge.

Requirements:

  • RN licensure in the State of Maryland
  • Current Cardio Pulmonary Resuscitation (CPR) certification
  • Prior experience in hospice care required
  • Certified Hospice and Palliative Nurse (CHPN) certification preferred
