We are seeking a skilled Licensed Practical Nurse (LPN) to join our team at IntelliChoice Home Care. As a LPN, you will provide one-on-one nursing care in the home to critical and complex patients, focusing on trach and/or vent patients of all ages.

Responsibilities:
  • Provide high-quality nursing care to patients in their homes
  • Work closely with patients, families, and healthcare teams to develop and implement individualized care plans
  • Administer medications, perform treatments, and provide wound care as needed
  • Monitor patients' conditions and report any changes or concerns to the healthcare team
  • Collaborate with respiratory therapists to provide comprehensive care to patients with trach and/or vent needs
Requirements:
  • Current LPN license
  • CPR certification (in-person training only)
  • TB skin test (current within the last 12 months)
