Job Summary

Haven HHC is seeking a highly skilled and compassionate Occupational Therapist to join our team in Kissimmee, FL. As a Home Health Care Specialist, you will play a vital role in maximizing the health and well-being of our patients, while maintaining their independence and dignity.

Key Responsibilities
  • Plan and Implement Care: Develop and implement individualized plans of care to restore motion and function, ensuring patients achieve their optimal level of independence and mobility.
  • Collaborate with Healthcare Team: Work closely with physicians, nurses, and other healthcare professionals to ensure seamless care coordination and effective treatment outcomes.
  • Conduct Initial Evaluations: Conduct thorough initial evaluations to assess patients' physical disabilities and develop targeted treatment plans to improve their quality of life.
  • Provide Education and Training: Educate patients and their families on home programs and activities of daily living, empowering them to take an active role in their recovery.
  • Document Progress: Maintain accurate and detailed clinical and progress notes, ensuring timely and effective communication with the healthcare team.
Requirements
  • Current License: Possess a current license to practice as an Occupational Therapist in the state of residence.
  • Home Health Experience: Minimum of two years' experience in home health care, with a strong understanding of treatment practices and patient care coordination.
  • Excellent Communication Skills: Demonstrate exceptional interpersonal and communication skills, with the ability to work effectively with patients, families, and healthcare professionals.
  • Continuous Learning: Possess a drive to continually learn and grow, staying up-to-date with the latest treatment practices and industry developments.
