Key Responsibilities:

  • Coordinate the RAI process to ensure accurate and timely completion of resident assessments in accordance with Medicare, Medicaid, OBRA, and other payor program requirements.
  • Ensure assessments accurately reflect the physical, mental, and psycho-social status of each resident.
  • Ensure appropriate documentation to report and support services provided and assessment accuracy.
  • Communicate effectively with other members of the interdisciplinary team.
  • Follow all Washington Square Nursing & Rehab policies and procedures.

Requirements:

  • LPN or RN licensure.
  • Strong communication and interpersonal skills.
  • Ability to work effectively in a fast-paced environment.
  • Knowledge of RAI process and payor program requirements.
