Oakwood Village West is seeking a skilled and compassionate Licensed Practical Nurse (LPN) to join our team in our Skilled Nursing Facility. As an LPN, you will play a vital role in providing high-quality care to our residents, working collaboratively with our interdisciplinary team to ensure the best possible outcomes.
Key Responsibilities- Contribute to the assessment, planning, implementation, and evaluation of resident care on a defined unit in the skilled nursing facility.
- Collaborate with nurses from all shifts, interdisciplinary staff, and physicians on issues related to the management of resident care.
- Perform resident care, including medication administration, completion of treatments, data collection, and basic teaching and reinforcement education.
- Document and report changes in resident condition, intervention, and resident response in coordination with RN.
- Update Care Plans consulting with RN as appropriate.
- Communicate Care Plan changes with Nursing and therapy staff.
- Attend Care conferences as designated.
- Report changes in resident condition to supervisor and seek guidance from RN as appropriate.
- Licensed practical nurse license required.
- One year relevant nursing experience in long-term care preferred.
- CPR Certified required.
