Responsibilities

Position Summary
This role is crucial in ensuring the safe and effective utilization of medications. Responsibilities include verifying medication orders while assessing optimal drug selection, dosage regimen design, effective administration, and monitoring drug therapy services within the health system.

Key responsibilities include:

  • Monitoring inpatient and outpatient drug therapies as appropriate
  • Collaborating with medical staff to identify alternative therapies when necessary
  • Evaluating patient care plans and resolving medication-related issues to optimize pharmaceutical care
Position Responsibilities
  • Review, interpret, and evaluate all medication orders for patients across all age groups.
  • Maintain patient medication profiles within an automated pharmacy information system.
  • Consult and advise medical, nursing, and other healthcare professionals regarding medication prescribing, dispensing, or administration.
  • Routinely monitor total drug therapy for effectiveness, side effects, adverse reactions, toxicities, drug interactions, and incompatibilities.
  • Conduct assigned target drug monitoring programs and concurrent drug use evaluations as required.
  • Perform and document Pharmacist Intervention activities, including recommendations on drug selection, dosage adjustments, and appropriate therapy.
  • Obtain medication histories for selected patients and communicate relevant information to healthcare providers.
  • Communicate with physicians, nurses, and other healthcare providers as necessary.
  • Prepare and/or verify the correct preparation of medications for dispensing, utilizing appropriate techniques.
Qualifications
Requirements:
- Registered Pharmacist with the Texas State Board of Pharmacy in good standing (RPH)
- Aseptic Technique - Completion of an ACPE approved 20-hour sterile products course

Part Time
Night Shift
Sunday - Thursday
9 PM - 7 AM every other week
