About the Role

We are seeking an experienced and highly skilled Hospitalist Nurse to join our dynamic and ever-growing healthcare team. As a key member of our cardiovascular care team, you will serve as the first point of contact for our patients, working closely with our team of cardiologists and advanced practice providers to deliver exceptional patient care.

Responsibilities
  • Patient Care:
    • Educate patients and their families on the role of the Hospitalist Program and verify primary care provider information.
    • Actively participate in morning report/hand-off and serve as census coordinator.
    • Collaborate with the hospital care team to ensure timely completion of current labs, test results, consults, orders, and other tasks.
    • Initiate proactive planning and coordination of the patient's plan for the day and stay with the rest of the team.
    • Participate in quality improvement initiatives, review core measure indicators, and ensure team goals are met.
    • Assess patient needs and collaborate to set up follow-up needs upon discharge.
    • Obtain medical records and assist with medication reconciliation.
    • Occasionally set up care team meetings with physicians and families to discuss ongoing hospital care, safe discharge plans, and disposition.
    • Serve as continuity for patients during physician hand-offs.
    • Collaborate with physicians and care coordinators regarding care delays.
  • Care Coordination:
    • Resolve post-discharge issues needing follow-up and provide post-discharge homecoming calls to follow-up with high-risk patients.
    • Educate patients at the bedside and over the phone, recognizing limits of when to collaborate with a physician regarding patient care.
    • Fill out documents for patients and families regarding hospital stay.
    • Follow-up on labs and other pending items post-discharge.
    • Coordinate care with patients until they have been seen by an established or new primary care provider.
Qualifications
  • Education: Graduate of an accredited program for Registered Nurses.
  • Experience: 3-5 years of nursing experience preferred.
  • License(s)/Certification(s): Current license to practice nursing in the State of Iowa for Registered Nurse, BLS, and Mandatory Reporter.
