Job Summary:

We are looking for a highly motivated and compassionate Occupational Therapist to provide high-quality care to our patients. As a Home Health Occupational Therapist, you will be responsible for assessing patients' needs, developing and implementing individualized therapy plans, and educating patients and their families on how to perform daily tasks and maintain their independence.

Responsibilities:
  • Assess patients' physical and cognitive abilities to identify areas for improvement
  • Develop and implement individualized therapy plans to help patients achieve their goals
  • Provide occupational therapy to patients in their own homes, including exercises to improve fine motor skills and coordination
  • Assist patients with adaptive equipment, such as grab bars and shower chairs, to improve safety and independence
  • Collaborate with other healthcare professionals, including RNs, LPNs, CNAs, HHAs, PTs, and SLPs, to ensure comprehensive care
  • Document patient progress and communicate with healthcare teams to ensure continuity of care
Requirements:
  • Graduate of an accredited Occupational Therapy Program
  • Active OT license in Michigan
  • Minimum of 1 year of occupational therapy experience, preferably in home healthcare
  • CPR certification
  • Knowledge of state and federal home health regulations
