We are seeking a highly skilled Physical Therapist to join our team at Cityview Nursing and Rehabilitation Center. The ideal candidate should have experience in Short Term Rehab and be familiar with updated codes for Skilled Care. The working hours are Friday to Monday, 8am to 5pm, with the potential for additional hours based on census. The provider will be required to log hours from EMR (Rehab Optima) and must stay busy in the building to meet the guaranteed hours. While there are no productivity requirements, clocking in and out is necessary to receive hours. The successful candidate will be responsible for providing high-quality physical therapy services to our residents, promoting their well-being and independence.
Key Responsibilities:- Provide physical therapy services to residents in a Skilled Nursing and Rehabilitation setting
- Develop and implement individualized treatment plans to meet the unique needs of each resident
- Collaborate with other healthcare professionals to ensure comprehensive care
- Log hours from EMR (Rehab Optima) and maintain accurate records
