Home Health Intake Coordinator

Eden Health is seeking a highly organized and detail-oriented Home Health Intake Coordinator to support our administrative office in Kirkland, WA.

This role assists with the coordination of patient care, including intake, scheduling, medical records, and reception work for coverage during the weekend (Friday through Sunday).

Key responsibilities include:

  • Data entry into the electronic health record (EHR)
  • Patient scheduling and ensuring patient satisfaction through consistency and continuity
  • Reviewing medical records for completeness and retrieving missing information
  • Attaching documents to patient files received via fax, scan, or mail into the EHR
  • Monitoring aged unsigned provider orders and submitting requests for signatures
  • Receiving and responding to requests for medical records and transporting/transferring records as needed
  • Reviewing Face-to-Face and referral documentation for completeness and adequacy
  • Answering and transferring phones efficiently
  • Using EHR to update patient demographic data accurately
  • Processing incoming and outgoing mail and faxes as needed
  • Assisting management with typing, filing, and computer-related tasks
  • Filing, printing, faxing, copying, cleaning, organizing, and assembling new patient folders
  • Reviewing patient charts for billing accuracy and following up on unmet requirements
  • Providing backup to other Eden team members as assigned
  • Participating in information management and quality assurance performance improvement activities

Requirements include:

  • High school diploma or equivalent
  • General clerical, data entry, and excellent organizational skills
  • Healthcare experience preferred, with medical terminology desired
  • EHR medical record documentation experience preferred
  • Ability to communicate effectively in English verbally and in writing
  • Working knowledge of computer software programs
  • Valid driver's license and reliable transportation
  • Able to pass a criminal background check and national sex offender clearance
