Transfer Center LPN/LVN

We are seeking a highly skilled and experienced Licensed Practical Nurse (LPN) or Licensed Vocational Nurse (LVN) to join our Transfer Center team as a Clinical Triage Nurse. This is a 100% remote position, and the ideal candidate will have a strong background in clinical triage, case management, and hospital admissions.
Key Responsibilities- Provide high-quality, prompt assistance with clinical triage and other duties as assigned.
- Monitor and respond to triage needs, completing nurse-to-nurse reports in a timely manner.
- Use technology to review patients' clinical needs and respond to referring facilities regarding possible admissions.
- Respond to faxed clinicals within 15 minutes with an admission decision or communicate delays to referring facilities.
- Collaborate with referral sources, physicians, nurses, and other healthcare professionals to provide quality care and support.
- Use crisis stabilization techniques and proper procedures to make referrals to appropriate facilities.
- Coordinate and correspond with hospital Assessment staff when accepting transfers.
- Enter demographics and chief complaint information into doc-to-doc systems.
- Respect confidentiality of callers and clients at all times.
- Communicate via Excel logs to track referrals managed during shifts.
- Valid Compact LPN/LVN licensure.
- At least three years of experience as an LPN/LVN, with previous experience in case management, hospitals, and admissions preferred.
- ER and ICU experience is preferred, as well as a minimum of 1-2 years of experience in a Behavioral Health facility.
- Valid driver's license preferred.
- Ability to work flexible hours and do overtime as needed.
