Job Title: Telehealth Nurse Specialist

The Telehealth Nurse Specialist will work as part of a multidisciplinary team to deliver telephonic triage services to patients. This role requires excellent communication skills, critical thinking, and the ability to make informed decisions quickly.

Key Responsibilities:

  • Provide telephonic triage services to patients, using established workflows and protocols to determine the most appropriate level of care needed.
  • Collaborate with customers to reduce readmissions, enhance chronic disease management, manage health risks, and report injuries.
  • Accurately document patient information in the Electronic Medical Record (EMR) according to best practices, guidelines, or workflows.
  • Participate in virtual monitoring and subsequent reporting, escalating concerns to support services as necessary.

Requirements:

  • RN license required in applicable state(s); multistate/compact RN licensure preferred.
  • 1 year of acute care nursing experience required; triage experience preferred.
  • ADN or diploma in nursing required; BSN preferred.
