As a key member of our healthcare team, you will provide high-quality patient care, working closely with patients, families, and healthcare professionals to achieve optimal outcomes.

Responsibilities:

  • Utilize the nursing process to assess, plan, implement, and evaluate patient care in a Long Term Care/Assisted Living Facility
  • Perform focused interviews to identify specific needs and develop individualized care plans
  • Assess signs and symptoms indicating physiologic and psychological changes in the patient's condition, documenting actual and/or potential nursing diagnoses
  • Collect, analyze, and interpret data from healthcare team members, implementing interventions according to identified priorities and facility policies
  • Provide a safe, comfortable, and therapeutic patient environment, initiating and evaluating patient and/or family education
  • Maintain accurate records, monitoring safety and effectiveness of the environment and equipment, adhering to regulatory requirements and professional standards

Requirements:

  • Current Registered Nurse License for the state in which the nurse practices
  • Compliance with all relevant professional standards of practice, including those related to confidentiality and HIPAA
  • Participation and completion of Amergis' Competency program when applicable, maintaining current CPR certification if necessary
  • Ability to communicate effectively with patients, families, and healthcare professionals, using strong verbal and written skills
  • Computer proficiency and ability to work in a fast-paced environment
