We are seeking a skilled and compassionate Registered Nurse to join our Home Health team. As a Home Health RN, you will provide high-quality patient care in the comfort of their own homes, working collaboratively with our interdisciplinary team to deliver exceptional outcomes.
Key Responsibilities- Conduct comprehensive initial assessments for each assigned patient, including physical, psychosocial, and functional evaluations
- Evaluate patient status and progress toward goals with each visit, revising the care plan as necessary
- Establish and implement individualized plans of care in collaboration with the patient, family, and healthcare team
- Initiate preventative and rehabilitative nursing procedures, coordinating Home Health services as needed
- Respond to patient needs holistically, addressing physical, emotional, spiritual, and family concerns
- Graduate of an accredited school of professional nursing, BSN preferred
- Current RN licensure in Ohio, with at least one year of medical/surgical nursing experience
- Home Health and/or community health experience preferred, with strong understanding of the nursing process
- Excellent communication and telephone skills, with ability to rotate on-call shifts
- Valid State of Ohio motor vehicle operator's license and insurable under Firelands' auto policy
