As an Advanced Practice Clinician, your primary responsibilities will include:

  • Conducting thorough home assessments, including medical history evaluations, medication reviews, and physical exams.
  • Performing evidence-based practice screenings and point-of-care testing as needed.
  • Identifying diagnoses and developing care plans to address gaps in patient care.
  • Communicating with patients' primary care physicians to ensure coordinated care.

You will have the flexibility to achieve a healthy work-life balance with flexible scheduling options. With approximately 85% travel requirements within the Mid-Atlantic region, you will have opportunities to explore new places while making a positive impact on patients' lives.

Our company offers a competitive salary range of $88,000 to $173,200 annually, based on factors such as education, experience, and certifications. In addition to a comprehensive benefits package, we provide incentive and recognition programs, equity stock purchase opportunities, and a 401(k) contribution plan.

To be successful in this role, you must possess:

  • An active NP license or ability to obtain by start date, with unencumbered licensure.
  • A minimum of 1 year of clinical experience in your highest level of education, preferably in Gerontology, Cardiology, Internal Medicine, or Endocrinology.
  • Ability to complete the physical demands of the job, including lifting a 30-pound bag and navigating various dwelling conditions.
