Job Title: Wellness Advocate

Trinity Alliance of the Capital Region is a leading social services organization dedicated to promoting healthy families and improving communities. As a Wellness Advocate, you will play a vital role in supporting the well-being and success of our clients.

Key Responsibilities:
  • Establish trusting relationships with clients and their families, providing culturally competent and trauma-informed support and encouragement.
  • Provide social care to clients, assisting with areas such as housing, food insecurity, financial stability, and other factors that impact healthcare access and outcomes.
  • Engage with clients, build relationships, navigate systems, advocate for clients, and provide health literacy instruction and outreach.
Requirements:
  • High school diploma or equivalent; some years of higher education preferred but not required.
  • At least 3 years of positive work history.
  • Strong computer skills required.
  • Bilingual preferred.
  • Excellent crisis intervention, case management, and advocacy skills.
  • Valid driver's license, insurance, and good driving record.
Benefits:
  • $22 per hour; 25 hours per week.
  • Paid time off pro-rated; sick leave and vacation leave.
