Job Overview:
We are seeking a skilled Licensed Practical Nurse (LPN) to provide high-quality care to patients in their homes. As a home care LPN, you will be responsible for creating personalized care plans, administering medications, performing wound care, and monitoring patients' conditions. You will also educate patients and their families on their conditions and provide emotional support.

Responsibilities:
- Create comprehensive care plans tailored to each patient's needs
- Administer medications as prescribed by physicians
- Perform wound care and dressing changes
- Monitor patients' conditions and report any changes to physicians
- Educate patients and their families on their conditions and provide emotional support

Requirements:
- Associate's Degree in Nursing
- Current BLS certification
- At least 1 year of LPN experience, with preference given to those with home care experience
- Ability to work independently and as part of a team
