Home Health Registered Nurse Job Description

BAYADA Home Health Care is seeking a skilled Registered Nurse (RN) to provide high-quality care to clients in their homes. As a Home Health Registered Nurse, you will be responsible for delivering patient-centered care, conducting thorough assessments, and developing personalized care plans.

Responsibilities:
  • Conduct home visits to assess clients' needs and develop care plans
  • Administer medications, wound care, and other treatments as prescribed
  • Monitor clients' conditions and report changes to the Clinical or Client Services Manager
  • Follow up with doctors' orders and execute necessary procedures
  • Accurately document observations, interventions, and evaluations using a state-of-the-art touch pad tablet
Requirements:
  • A current license as a Registered Nurse in VA
  • A minimum of one year of recent, verifiable clinical (medical/surgical) experience
  • Prior home care experience is strongly preferred, but not required
  • Graduation from an accredited and approved nursing program
  • Ability to work independently and manage time effectively
  • Strong interpersonal skills
  • Solid computer skills, with prior experience with electronic medical records (EMR) preferred
