Job Summary:

The North Colorado Health Alliance is seeking a skilled Healthcare Manager to join our team. As a key member of our organization, you will play a critical role in coordinating care and connecting individuals to valuable resources in the community.

Key Responsibilities:

  • Collaborate with local healthcare providers, community agencies, and members to coordinate care and improve health outcomes.
  • Conduct needs assessments and develop person-centered care plans to address the unique needs of each individual.
  • Assist members in scheduling appointments, follow-up care, referrals, and medication refills.
  • Attend member appointments with providers and community resources as needed.
  • Participate in health promotion and education activities for members.
  • Establish collaboration and communication among all responsible parties of an individual member's multidisciplinary healthcare team.
  • Work in conjunction with various providers to develop interventions as needed.
  • Optimize member and family self-management through education, community resources, and support.
  • Review and reduce duplication of services for members.
  • Enter all documentation, contacts, and assessments into multiple databases.
  • Timely responsiveness to requests and communications.
  • Understand, communicate, and facilitate member complaints, grievances, and appeal processes.
  • Regular case review with supervisor.
  • Maintain strict confidentiality of member records and communications.

Requirements:

  • High School Graduate or GED required.
  • Valid Colorado Driver License and Insurance required.
  • Strong decision-making skills, cultural competence, and critical thinking skills.
  • Excellent verbal communication skills and ability to work independently in a busy office environment.
  • Ability to manage stressful situations and utilize positive problem-solving skills.
  • Computer skills, including Microsoft Office and state-administered data collection systems.
  • Experience working with Medicaid and Medicare, and partnering with local and regional health and human service resources.
  • Clinical skills, including effectively engaging with multicultural populations and addressing various medical, behavioral health, and social issues.
