We are a private practice in Suffolk County, seeking an experienced Waiver Service Navigator to work with NHTD and TBI participants in different regions.

Pay Rate

$45.00 - $55.00 per case, paid on a monthly basis.

Job Duties

The Waiver Service Navigator is responsible for providing comprehensive case management services to clients, including needs assessment, service plan development, coordination of services, and monitoring of progress towards goals.

The ideal candidate will have experience working with individuals with disabilities and a strong understanding of the NHTD and TBI waiver programs.

Key Tasks:

  • Conduct comprehensive needs assessments with clients to identify their goals, strengths, and barriers to success.
  • Develop revised service plans that are client-centered, strength-based, and goal-oriented.
  • Coordinate with service providers and community resources to ensure clients receive the services and support they need.
  • Monitor client's progress towards their goals, adjust service plans as needed, and document progress in case files.
  • Provide crisis intervention and support to clients as needed.
  • Maintain accurate electronic case records and documentation in accordance with agency and regulatory requirements.
  • Participate in team meetings, case conferences, and trainings as required.
  • Provide referrals to other services and resources as needed.

Education and Experience:

  • Associates degree with 5 years of NHTD/TBI waiver service coordination experience.
  • Bachelor's degree with 3 years of service coordination experience.
  • Master's degree with one year of service coordination experience.
