Job Summary
We are seeking a skilled Hospice Licensed Practical Nurse (LPN) to join our team. As a Hospice LPN, you will play a vital role in delivering high-quality patient care, collaborating with our interdisciplinary team, and providing emotional support to patients and their families.
Key Responsibilities
- Provide direct, skilled nursing services to patients in accordance with their individualized Plan of Care.
- Collaborate with the RN Case Manager and other healthcare professionals to ensure comprehensive care.
- Build trusting relationships with patients and their families, providing emotional support and education on care and comfort measures.
- Empower caregivers by educating them on care and support strategies for their loved ones.
- Participate in interdisciplinary team meetings to discuss patient care and develop care plans.
Requirements
- Current LPN licensure in the state of practice.
- Experience in hospice care or a related field.
- Strong communication and interpersonal skills.
- Ability to work collaboratively as part of a team.
