Position: Per Diem RN Wellness Nurse

Phoebe Ministries is seeking a dedicated Per Diem RN Wellness Nurse to contribute to our long-term care services. We value our staff's commitment to enhancing the quality of life for seniors and maintain a high standard of excellence within our facilities.

Key Responsibilities:

  • Support the Director in managing and coordinating the holistic wellness initiatives for residents in independent living.
  • Conduct clinical, cognitive, and social evaluations of residents at designated intervals.
  • Facilitate referrals for support services and provide necessary education.
  • Coordinate connections to fitness, activity, and pastoral care programs as needed.
  • Participate in interdisciplinary wellness meetings to analyze assessment data and develop comprehensive support plans.
  • Deliver a range of nursing duties to ensure high-quality care for residents.

Qualifications:

  • Graduate from an accredited nursing program; possess a valid Pennsylvania RN or LPN license.
  • At least one year of experience in relevant roles preferred.
  • Strong skills in reading, writing, and mathematics; excellent interpersonal and communication abilities; knowledge of medication interactions and side effects; current CPR certification.
  • Ideal candidates will thrive in a respectful, customer-focused culture and embody the organization's mission and values.
