Key Responsibilities:
- Leadership and Management: Lead and guide the Case Management team, including management, medical staff, resource, and clinical staff, in planning and conducting organization-wide case management activities.
- Oversight and Coordination: Provide oversight of case management, discharge planning, and utilization review to ensure seamless transitions and optimal patient outcomes.
- Strategic Planning: Participate in operations to ensure comprehensive departmental services compliant with regulatory requirements and promote a collaborative departmental approach to strategic operational planning.
- Performance Improvement: Continuously assess, measure, and improve departmental performance by managing departmental resources responsibly and ensuring staff professional needs are met.
Requirements:
- A minimum of 3 years of experience in leadership with a high-volume case management program in a hospital or clinic setting.
- Current Registered Nurse Licensure and Bachelor of Science in Nursing required.
- Case management leadership experience is essential.
