Job Description

As a Home Healthcare RN, your primary responsibility will be to provide skilled nursing care to patients in their homes. You will work collaboratively with our multidisciplinary team to develop and implement individualized care plans that address the unique needs of each patient.

  • Provide hands-on nursing care to patients in their homes, including wound care, medication management, and vital sign monitoring.
  • Conduct thorough assessments of patients' physical and emotional needs, identifying areas for improvement and developing targeted interventions.
  • Collaborate with other healthcare professionals, including physicians, therapists, and social workers, to ensure comprehensive care planning.
Requirements

To be successful in this role, you must possess:

  • A current Registered Nurse (RN) license in the state of Ohio.
  • At least one year of prior experience as a Registered Nurse, preferably in a home healthcare setting.
  • Current Health Certificate, PPD, and BLS certification.
