We are seeking a skilled and compassionate physician to join our team in Central Massachusetts. As a member of our interdisciplinary team, you will play a vital role in providing high-quality care to our patients.
About the Role
This is a unique opportunity to work in a PACE program, which provides a combination of medical, social, and insurance services to seniors. Our program is designed to help patients avoid nursing home placement and maintain their independence.
Key Responsibilities
- Provide outpatient medical care to patients with complex and frail conditions
- Work collaboratively with an interdisciplinary team to develop and implement individualized care plans
- Offer comprehensive insurance coverage with no co-payments or paperwork for participants
- Have the flexibility to devote as much time as needed for the clinical and social needs of patients and their caregivers
- Participate in the Interdisciplinary Team (IDT) approach to care planning, which is the gold standard for complex and frail seniors
