About the Role

We are seeking a skilled Nurse Practitioner (NP) or Physician Assistant (PA) to join our HouseCalls team as a Statewide Traveler. As a key member of our healthcare team, you will provide high-quality patient care through in-home assessments and travel across various locations.

Compensation Package

As a valued member of our team, you can expect a competitive compensation package, including a generous $40,000 Student Loan Repayment or $40,000 Sign-on Bonus for individuals who have not previously participated in this program.

Job Description

As a HouseCalls Statewide Traveler, your primary responsibilities will include:

  • Conducting in-home assessments, including evaluating past medical history, reviewing systems, medication reconciliation, vital signs, and comprehensive physical exams.
  • Performing evidence-based practice screenings, including point-of-care testing (as appropriate).
  • Identifying diagnoses to be used in care management and active medical management in the furtherance of treatment.
  • Formulating a list of current and past medical conditions using clinical knowledge and judgment and the findings of your assessment.
  • Communicating findings in your assessment that will be used to inform members' PCPs of potential gaps in care.
  • Identifying urgent and emergent situations and intervening appropriately.
  • Educating members on topics such as disease process, medication, and compliance.
Requirements and Qualifications

To be successful in this role, you must possess:

  • An active NP or PA license or ability to obtain by start date. Licensure must be unencumbered, free of any open/unresolved disciplinary actions including probation or restrictions against privilege to practice.
  • An active national NP or PA certification or the ability to obtain national certification in the state of assignment by start date (For NPs - Active ANCC or AANP national certification in Family, Geriatrics, Adult, Adult-Gerontology Primary Care, or Adult-Gerontology Acute Care certification OR for PAs - Physician Assistant national certification through NCCPA).
  • The ability to complete physical requirements of the job, including lifting a 30-pound bag in and out of the car, navigating stairs and a variety of dwelling conditions, and sit, stand, and kneel to perform physical assessment.
  • Reliable transportation to complete home visit assessments.
