We are seeking a skilled Licensed Vocational Nurse (LVN) to join our Hospice Care team at MemorialCare Health System. As a Hospice LVN, you will play a vital role in providing high-quality patient care and support to individuals and their families during a challenging time.

• Assess and Evaluate Patient Needs: Conduct thorough assessments of patients' physical, emotional, and social needs to develop effective care plans.
• Coordinate Care Services: Collaborate with healthcare teams, including physicians, nurses, and social workers, to ensure seamless care delivery and coordination.
• Provide Symptom Management and Education: Teach patients and their families how to manage symptoms, promote comfort, and improve quality of life.
• Document Patient Information: Accurately and thoroughly document patient information, including medical history, treatment plans, and progress notes.
• Communicate Effectively: Develop and maintain strong relationships with patients, families, and healthcare teams through effective communication and empathy.

• Education and Experience: Minimum of 2 years of experience as a Licensed Practical Nurse (LVN) and a current California license.
• Certifications and Training: BLS CPR certification and completion of hospice-specific training programs.
• Personal Qualities: Dependable transportation, a valid driver's license, and car insurance.
