We are seeking a skilled and compassionate Nurse Practitioner to join our team at Matrix Medical Network. As a member of our clinical staff, you will have the opportunity to work with older adults and other at-risk individuals in their homes, providing comprehensive health assessments and improving quality of care.

Estimated Salary: $92.50 per hour

About the Role

This is a part-time position that offers flexible scheduling and the opportunity to make a direct impact on the health and well-being of others. As a Nurse Practitioner with Matrix Medical Network, you will be responsible for conducting adult and geriatric assessments, medical history reviews, and developing treatment plans tailored to each patient's needs.

Your Key Responsibilities:

  • Conduct thorough health assessments, including review of medical history, diagnosis, and treatment recommendations.
  • Work collaboratively with healthcare providers, social workers, and family members to develop comprehensive care plans.
  • Provide education and support to patients and their caregivers on managing chronic conditions and promoting healthy lifestyles.
  • Collaborate with Primary Care Physicians (PCPs) on patient education and follow-up care.

Requirements:

  • Masters degree in nursing or equivalent experience and satisfactory completion of NP licensure.
  • Current RN and NP licensure in the state of practice, including prescription authority.
  • Board certification by AANP, ANCC, or AACCN in a Matrix-approved specialty.
  • Minimum 1 year experience as a Nurse Practitioner.
  • Excellent verbal and written communication skills, with ability to work effectively with patients, clients, and colleagues.
