About the RoleWe are seeking a skilled and compassionate Licensed Vocational Nurse (LVN) to join our team of healthcare professionals. As a Home Health LVN, you will be responsible for administering quality, skilled nursing care to patients in their homes. This will involve working in accordance with physician orders and the Plan of Care under the direction and supervision of a Registered Nurse and/or Director, Professional Services.
Key Responsibilities- Administer skilled nursing care to patients in their homes, as ordered by physicians and outlined in the Plan of Care.
- Work collaboratively with other healthcare professionals to ensure the provision of quality patient care.
- Provide patient education and support to promote health and wellness.
- Document patient care and progress in a timely and accurate manner.
- Communicate effectively with patients, families, and healthcare team members to ensure seamless care coordination.
- Graduate of an accredited LPN/LVN program.
- Minimum of one year experience as an LPN/LVN in a clinical care setting.
- Home care experience preferred.
- Current BCLS certification prior to delivering patient care.
