Key Responsibilities:
  • Effectively screen and evaluate patients with communication, cognitive, or swallowing disorders
  • Develop and implement individualized plans of care
  • Provide rehabilitative, skilled, and medically necessary treatment interventions
  • Evaluate treatment outcomes and modify services as needed
  • Provide consultation and counseling to patients, families, caregivers, and other service providers
  • Document patient information and maintain accurate records
