Work Schedule: Day shifts, Monday to Friday

Position Overview:
  • The Physical Therapist is responsible for developing and implementing tailored physical therapy programs while guiding other staff and family members in specific therapy phases.
Key Responsibilities:
  • Deliver physical therapy interventions based on a physician's established care plan.
  • Conduct assessments to evaluate the therapeutic and functional status of patients, contributing to the comprehensive care plan.
  • Evaluate the home environment to ensure it supports rehabilitation efforts.
  • Assess muscle strength, mobility, gait, and range of motion, focusing on comfort for hospice patients.
  • Oversee the administration of physical therapy treatments.
  • Educate patients and caregivers on the proper use and maintenance of therapeutic devices.
  • Identify and prioritize the physical therapy needs of patients.
  • Communicate any changes in patient condition or treatment responses to the physician.
  • Provide training for patients on the use of prosthetic devices when applicable.
  • Recognize additional home health or hospice service needs and make referrals as necessary.
  • Prepare and submit clinical documentation and progress reports based on goal achievement.
  • Engage in discharge planning for patients.
  • Facilitate in-service training sessions for nursing staff as required.
  • Participate in peer review and consultation processes.
Qualifications:
  • Current BLS/BCLS certification.
  • Valid Physical Therapy license in the state of Washington.
  • New graduates are welcome to apply.
  • Experience in skilled nursing facilities is preferred.
  • Completion of a COVID-19 test within the specified timeframe prior to the start date.
