Certified Nursing Assistant Hospice

We are seeking a dedicated and compassionate Certified Nursing Assistant to join our hospice team at Medi Home Hospice. As a member of our interdisciplinary team, you will provide essential support services to patients and their families, promoting physical and emotional comfort in their homes.
The ideal candidate will possess a strong commitment to delivering high-quality care, excellent communication skills, and a passion for working with patients and families in a hospice setting. Responsibilities:
- Provide personal care and activities of daily living assistance to patients, as outlined in their plan of care.
- Apply safety principles and proper body mechanics to ensure the well-being of patients during personal care and supportive services.
- Prepare and provide adequate nutrition and fluid intake, as prescribed by medical professionals.
- Observe and report changes in patient condition or expectations of service promptly to skilled nurses via telephone and document communication in the Electronic Medical Record (EMR).
Requirements:
- High school diploma or General Education Degree (GED) required.
- Documented completion of a state-recognized Certified Nursing Assistant course.
- Valid Certified Nursing Assistant certification as required by the state.
- Valid driver's license and company-required auto-liability insurance.
